Editorial Commentary: Fixation Matters-When Technique Becomes Biology in Meniscal Allograft Transplantation

Abstract:
Meniscal allograft transplantation is a well-established treatment for symptomatic meniscal deficiency, yet the optimal fixation strategy remains debated. Recent studies revealed that suture-only fixation is associated with significantly higher meniscus-specific reoperation rates compared with bone plug and bone bridge-in-slot techniques. These findings reinforce that the fixation method is not merely a technical consideration, but a determinant of graft biomechanics and survivorship. Suture-only fixation may inadequately restore native hoop stress and tibiofemoral contact mechanics, contributing to increased graft extrusion, elevated joint pressures, and subsequent reoperation. Additionally, variation in outcomes by compartment and patient sex suggests that both anatomic and biological factors interact with fixation strategy. These data challenge the notion that fixation techniques are interchangeable and instead position fixation as a modifiable factor influencing clinical outcomes. Surgeons should incorporate these findings into operative decision-making and patient counseling, favoring fixation strategies that more reliably restore native meniscal function and improve long-term graft durability.
