Redefining Periodontal Care: Innovative Solutions for Diverse Populations

Abstract:
Periodontal diseases remain highly prevalent worldwide and represent a significant public health concern, particularly among special populations. These groups, including geriatric and pediatric patients, pregnant women, individuals with disabilities, medically compromised patients, and socioeconomically disadvantaged populations, experience a disproportionately higher burden of disease due to complex interactions between systemic, behavioral, and environmental factors. This review provides a comprehensive overview of periodontal health in these diverse populations, highlighting key risk determinants, clinical challenges, and contemporary management strategies. Aging-related changes such as immunosenescence, polypharmacy, and functional limitations increase susceptibility to periodontal disease in elderly individuals, while pediatric populations are influenced by developmental, genetic, and behavioral factors. Hormonal fluctuations during pregnancy exacerbate inflammatory responses, and systemic conditions such as diabetes and cardiovascular diseases further complicate periodontal outcomes. In addition, disparities in access to care, socioeconomic constraints, and cultural factors significantly contribute to disease progression and inequities in oral health. The review emphasizes the importance of preventive strategies, early intervention, patient education, and caregiver involvement. Emerging approaches, including minimally invasive therapies, laser-assisted treatments, tele-dentistry, and personalized care models, demonstrate potential in improving treatment outcomes and accessibility. A multidisciplinary and patient-centered approach integrating oral and systemic healthcare is essential for effective management. In conclusion, redefining periodontal care for special populations requires tailored, innovative, and equitable strategies that address both clinical and social determinants of health, ultimately improving periodontal outcomes and quality of life.
