Uncommon yet critical: Immediate hypersensitivity reactions to Gadobutrol in magnetic resonance imaging procedures

Abstract:
Gadolinium-based contrast agents are widely used in magnetic resonance imaging due to their excellent safety profile and tolerability. Hypersensitivity reactions to gadolinium-based contrast agents are uncommon, but they may occur with varying severity. The authors present 2 confirmed cases of immediate hypersensitivity reactions to Gadobutrol, a macrocyclic nonionic gadolinium-based contrast agent. The first case involved a female patient with generalized pruritus, tachycardia, and nasal congestion following a magnetic resonance imaging scan, which resolved spontaneously. The second case involved a male patient with multiple comorbidities who experienced anaphylactic shock during a cardiac magnetic resonance imaging. In both cases, the diagnosis of hypersensitivity reaction was supported by the results of skin tests and basophil activation tests. The article discusses the importance of understanding the mechanisms of hypersensitivity reactions and the role of allergy work-up. It highlights the importance of thorough patient evaluation, considering the impact of comorbidities and medications on hypersensitivity reactions and treatment. Further research is needed to improve diagnostic approaches and understand the immunological mechanisms involved in gadolinium-based contrast agent-induced hypersensitivity reactions.
