Late Reposition of a Dislocated Toric Intraocular Lens in a Fibrosed Capsular Bag: A Case Report

Introduction:
Toric intraocular lenses (IOLs) are widely used to correct corneal astigmatism during cataract surgery. However, postoperative rotation or decentration may significantly compromise visual outcomes.
Case Presentation:
A 70-year-old patient presented with progressive visual deterioration several years after cataract surgery with toric IOL implantation. Examination revealed a rotated and partially dislocated toric IOL with significant residual astigmatism (-7.50 D). Intraoperatively, the nasal haptic was located in the sulcus and fibrotic adhesions within the capsular bag were present. After careful viscodissection and release of capsular adhesions, the IOL was mobilized and repositioned into the capsular bag using a push-pull technique and aligned to the intended axis.
Conclusion:
Several years after cataract surgery, a repositioning of a dislocated toric IOL is still possible and can restore visual function avoiding unnecessary complex IOL exchange procedures.
