A Body Shape Index and Risk of Kidney Function Decline in Older Adults with CKD

Higher A Body Shape Index is linked to kidney disease progression in older men, but not women. This study examined Body Shape Index and BMI in relation to kidney outcomes in older adults with chronic kidney disease (CKD).

- A nationwide longitudinal observational study included 54,459 individuals aged 65-74 years with CKD.
- Exposures included ABSI and BMI; the primary outcome was a 40% decline in estimated glomerular filtration rate (eGFR).
- Cox regression and restricted cubic spline analyses assessed associations, stratified by sex, with eGFR trajectory evaluated using mixed-effects models.
Main Results:
- Among 31,292 men and 23,167 women, 544 men and 315 women experienced a 40% eGFR decline.
- In men, higher ABSI quartiles (Q3, Q4) were associated with increased risk of kidney outcome (aHRs 1.32-1.72) and a faster eGFR decline.
- No significant association was found between high ABSI and kidney outcomes in women, nor between high BMI and kidney outcomes in either sex.
Conclusions:
- Elevated ABSI is associated with CKD progression in older Japanese men.
- The association between ABSI and kidney outcomes is less pronounced in older women with CKD.
- BMI was not found to be a significant predictor of kidney outcomes in this cohort.
