Mixed reality exposure and response prevention (MERP) offers a novel approach for obsessive-compulsive disorder (OCD). This case study shows MERP effectively reduced symptoms in a patient with chronic contamination fears, demonstrating its clinical feasibility.
Area of Science:
- Psychiatry
- Psychology
- Human-Computer Interaction
Background:
- Traditional exposure and response prevention (ERP) for obsessive-compulsive disorder (OCD) faces implementation barriers.
- Mixed reality exposure and response prevention (MERP) presents an innovative alternative to traditional ERP.
Purpose of the Study:
- To illustrate the clinical application and feasibility of MERP in a patient with chronic contamination-related OCD.
- To assess the acceptability and effectiveness of MERP in reducing OCD symptoms and improving daily functioning.
Main Methods:
- A case report of a 57-year-old woman with contamination-related OCD who received six therapist-led MERP sessions.
- MERP involved superimposing virtual contamination cues onto the real therapy environment for immersive exposure.
- The Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) was used to measure symptom severity.
Main Results:
- MERP was well-accepted with no adverse events reported.
- Symptom severity decreased from severe to moderate (Y-BOCS 26-20).
- Improvements were noted in obsessions, avoidance, neutralization, and patient-reported sense of agency.
Conclusions:
- MERP is a feasible and acceptable delivery format for ERP in contamination-related OCD.
- The study highlights challenges in MERP implementation, such as immersion fluctuations and technical limitations, and suggests practical strategies.
- MERP demonstrated successful transfer of exposure principles into daily life for the patient.
