Intraoperatively, in a patient with a history of macroadenoma, the cavernous segment of the internal carotid artery was injured. CTA revealed a dissecting pseudoaneurysm at the carotid bifurcation. This case illustrates a fulminant form of DIPH occurring within days of FD deployment.
