  • Key endpoints were modified Rankin Scale (mRS) scores (0-1), symptomatic intracranial hemorrhage (SICH), and mortality. Study quality was assessed using ROB2, and risk ratios (RRs) were calculated.

  • Out of 5912 retrieved articles, only four trials met the inclusion criteria.
  • EVT demonstrated a non-significant trend towards improved mRS scores (RR 1.09; 95% CI 0.84-1.42; P=0.54).
  • A non-significant increase in SICH (RR 1.54; 95% CI 0.48-5.00; P=0.47) and no difference in mortality (RR 1.00; 95% CI 0.55-1.79; P=0.99) were observed with EVT.

Conclusions:

  • Current evidence suggests endovascular therapy does not provide a significant benefit for MeVO strokes.
  • The existing data are inconclusive, highlighting the need for further high-quality research to establish optimal treatment strategies for MeVO strokes.
