Longitudinal Evaluation of a Low Glycemic Load Dietary Intervention: A Multicentric Observational Study in India

Introduction:
Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder marked by insulin resistance and progressive β-cell dysfunction. Medical Nutrition Therapy (MNT) plays a key role in improving glycemic control and reducing comorbid risks. This longitudinal, multicentric observational study assessed the impact of MNT using Diabexy Atta, a culturally tailored low glycemic load (GL) flour, on clinical and biochemical outcomes in real-world Indian settings.
Materials And Methods:
A trained team, supervised by a diabetes specialist, supported participants with home-based assessments and ongoing consultations. Recruitment began in July 2023, with the final visit in January 2025. Of 568 enrolled adults, 512 completed the study. Participants were followed quarterly to monitor glycosylated hemoglobin (HbA1c), FBG, PPBG, liver and kidney function, medication use, and diabetes-related complications. All received personalized diets with Diabexy Atta, and statistical analysis assessed clinical and biochemical changes.
Results:
Statistical analysis showed significant improvements in glycemic control. Mean HbA1c declined from 8.68% to 6.50% (P < 0.00001), with parallel reductions in FBG and PPBG. Medication use reduced from 59.6% to 41.4%, and complication rates from 28.5% to 12.0% in participant. There was no significant effect on kidney or liver function tests, except for a slight decrease in serum sodium among electrolytes.
Discussion:
This study suggests that incorporating Diabexy Atta, a culturally familiar low glycemic index food, into MNT is feasible and may improve glycemic and metabolic outcomes in adults with T2DM. Larger and longer studies are needed to confirm these preliminary findings and assess long-term effects.
Conclusion:
Incorporating Diabexy Atta, low-glycemic-load food, into structured medical nutrition therapy may improve glycemic and metabolic outcomes in adults with Type 2 Diabetes.
