    Summary

    Angiolymphoid hyperplasia with eosinophilia (AHE) presents as a reactive inflammatory lesion. New evidence suggests AHE may stem from immunologic injury, challenging its classification as a vascular neoplasm.

    Background:

    • Angiolymphoid hyperplasia with eosinophilia (AHE) is characterized by endothelial cell proliferation and inflammatory cell infiltration.
    • Older AHE lesions may exhibit lymphoid hyperplasia with germinal centers and associated blood eosinophilia.

    Observation:

    • A young man developed deep nodules on his hand and forehead, diagnosed histologically as AHE.
    • Direct immunofluorescence revealed IgA, IgM, and C3 deposits in small vessels of the forehead tumor.
    • Serum analysis detected IgA, IgM, and IgG cryoglobulins.

    Findings:

    • The presence of IgA, IgM, C3, and cryoglobulins in an AHE patient suggests an immune-mediated process.
    • Histopathological and immunofluorescence findings support an inflammatory, reactive etiology for AHE.

    Implications:

    • This case challenges the traditional view of AHE as a true vascular neoplasm.
    • Findings support the hypothesis that AHE is a reactive lesion secondary to immunologic injury.
