Abstract:
The 9th edition of the TNM classification represents a refinement of the staging system for oropharyngeal carcinoma. Based on the distinction between HPV-associated and HPV-negative tumors introduced with the TNM-8 classification system, TNM-9 aims to capture the prognostically relevant heterogeneity within the HPV-associated subgroup more precisely. In particular, greater emphasis is placed on nodal disease characteristics, including extranodal extension, to improve prognostic discrimination.
Abstract:
This CME article reviews the key factors of the TNM-9 classification for HPV-associated oropharyngeal carcinomas and critically discusses their clinical implication. In addition to formal aspects of staging, diagnostic work-up, the role of imaging, the significance of p16 immunohistochemistry as a surrogate marker, and the impact of smoking and comorbidity are addressed. Cervical carcinoma of unknown primary and nasopharyngeal carcinoma are highlighted as paradigmatic models of biologically integrated staging.
Abstract:
The aim of this article is to clarify the prognostic role of TNM-9 while outlining its limitations. Appropriate application requires standardized diagnostic procedures and interdisciplinary interpretation; therapeutic decisions should not be derived from TNM stage alone.
