The Training Gap in Endoscopic Spine Surgery: Learning Curves, Credentialing, and the First University Certificate

Study Design:
Narrative review and description of a university certificate program.
Objective:
To set out the rationale, structure, and assessment framework of the first university-level graduate qualification in endoscopic spine surgery, and to argue the need for formal credentialing as a precondition for safe adoption.
Summary Of Background Data:
Full-endoscopic spine surgery has expanded rapidly, supported by randomized and observational evidence of outcomes at least equivalent to conventional techniques. The technique carries a long learning curve, with reported inflection points between 20 and 72 cases. Most published series originate from highly experienced centers. Only 14% of practicing endoscopic spine surgeons have completed a formal fellowship, and 69% of surgeons regard existing credentialing arrangements as too lax to protect patients during skill acquisition.
Methods:
We reviewed the literature on learning curves, training provision, and credentialing in endoscopic spine surgery, together with comparative evidence from other surgical disciplines.
Results:
The Certificate of Advanced Studies in Spinal Endoscopy at the University of Zurich comprises six sequential modules totaling 15 ECTS credits: e-learning with simulator training, basic and advanced cadaver courses with directly observed wet-lab assessment, and three two-week supervised international fellowships assessed by annotated video case submission. Case progression is governed by a validated two-dimensional complexity classification derived from 112 surgeons in 29 countries. The platform is open: equivalent external courses may be credited toward the certificate, provided one module is completed in Zurich and all competency assessments are conducted by university-appointed faculty.
Conclusions:
Structured, objectively assessed, academically accredited training addresses a documented gap between the diffusion of endoscopic spine surgery and the infrastructure available to teach it. The model is transferable to other technically demanding procedures.
