The Evolving Landscape of Orthopaedic Research Fellowships: A Narrative Review and Strategic Framework for Excellence

Abstract:
The orthopaedic surgery residency match remains among the most competitive in graduate medical education, with match rates declining from 70% in 2016 to 61% in 2024. An increasing number of applicants pursue research year fellowships to strengthen their candidacy, yet no standardized curriculum exists to guide these programs. This narrative review synthesizes the available evidence on research year fellowships for orthopaedic surgery applicants and proposes an evidence-based curricular framework. Research gap years are pursued by two distinct populations: medical students seeking a dedicated research year during training (typically between their third and fourth years) to strengthen a first-time application, and previously unmatched applicants pursuing a fellowship after an unsuccessful match cycle. Although these groups overlap in fellowship structure, they may differ in baseline academic profile, application strategy, and expected match outcomes. First-time applicants completing research gap years demonstrate match rates of 73% to 91%, with success most strongly associated with research productivity rather than participation alone. Reapplicants who matched after a research gap year had accumulated markedly more publications than those who did not. Programs with high match rates share common features: dedicated faculty mentorship, structured productivity expectations, clinical exposure, didactic enrichment, and comprehensive application support. Notably, many reapplicants who matched did so at their research year institution, underscoring the importance of institutional relationships. We propose a standardized six-domain curriculum encompassing research productivity, clinical exposure, didactic enrichment, application enhancement, professional development, and evaluation. This framework serves both prospective fellows seeking structured guidance and program directors establishing or refining research year programs.
