Disadvantaged children in rural communities in the United States: A concept analysis

Objective:
To clarify the concept of disadvantaged children in rural communities (DCRC).
Method:
A concept analysis was conducted using Walker and Avant's eight-stage method. A rapid review of U.S.-based literature published between 2021 and 2025 was completed. Nineteen articles met inclusion and exclusion criteria. The Social Determinants of Health framework guided data extraction and synthesis to identify defining attributes, antecedents, consequences, and empirical referents of DCRC.
Results:
The analysis revealed substantial variability in how DCRC is defined and operationalized across studies. Five defining attributes were identified: economic instability; geographic and structural rurality; limited access to healthcare, education, and essential services; constrained community and social resources; and exposure to systemic inequities. Antecedents included rural residence, structural underinvestment, household vulnerabilities, and limited community infrastructure. Consequences encompassed health disparities, inequitable healthcare access, educational challenges, and long-term developmental inequities. Empirical referents were inconsistently applied and commonly focused on economic indicators rather than multidimensional measures.
Conclusion:
DCRC remains a concept in development with fragmented and inconsistent usage across disciplines. Applying a structured concept analysis anchored in the Social Determinants of Health framework provides greater conceptual clarity and a comprehensive definition that captures the complexity of disadvantage experienced by rural children.
Implications:
A clarified conceptualization of DCRC will strengthen nursing research, improve measurement consistency, and inform targeted interventions and policies reducing health inequities among children in rural communities.
