Development and application of older adult safety and ethics simulation education for nursing students: A

Background:
It is important to protect patients' rights by preventing the misuse and abuse of physical restraints through societal and ethical discussions and education.
Aim:
This study aimed to develop and implement a simulation-based education program on older adult safety and ethics for nursing students; determine the acceptability of the simulation design; examine pretest-posttest changes in attitudes toward the use of restraints, ethical sensitivity, and person-centered nursing; and explore nursing students' learning experiences during the simulation.
Design:
This convergent mixed-methods study analyzed both quantitative and qualitative data.
Setting:
University in South Korea.
Participants:
A total of 45 fourth-year nursing students were recruited.
Methods:
A simulation involving an older adult experiencing postoperative confusion, including postoperative nursing care, physical restraint application, and geriatric nursing ethics, was developed and implemented. Attitudes toward the use of physical restraints, ethical sensitivity, and person-centered nursing were measured and analyzed using SPSS for Windows, version 29.0. Qualitative content analysis was conducted to explore the learning experiences of nursing students during the simulation.
Results:
A significant pretest-posttest increase was observed in attitudes toward the use of physical restraints. No significant pretest-posttest changes were observed in ethical sensitivity or person-centered nursing. Qualitative content analysis revealed the following learning experiences: "Perceived Benefits and Challenges of Simulation," "Self-reflection and Awareness of Competence in Physical Restraint Care," and "Expanded Awareness of Patient Rights and Ethical Values."
Conclusion:
Older adult safety and ethics simulation may provide an educational context for addressing restraint-related ethical issues and person-centered nursing. Future studies should examine repeated and diverse simulation experiences.
