Practical Guidance for the Use of Control-IQ Technology During Physical Activity and Exercise

Introduction:
Use of automated insulin delivery (AID) systems during physical activity and exercise (PA) is safe and associated with improved glucose outcomes when compared with insulin pump therapy in manual mode and insulin pen therapy, provided that appropriate adjustments to insulin doses and carbohydrate intake are ensured.
Aim:
This expert guidance aimed to support healthcare professionals in educating Control-IQ (Tandem Diabetes Care) users on PA management.
Methods:
Based on international recommendations, including the recent EASD/ISPAD position statement on AID and exercise, as well as on the authors' clinical experience, we developed practical flowcharts and accompanying information that provide step-by-step guidance for PA management in children, adolescents and adults with type 1 diabetes using the Control-IQ technology.
Results:
Operational instructions were outlined across four different scenarios, with additional advice on insulin pump suspension where required. In each of the scenarios, we highlighted the possibility of activating a Personal Profile with more or less insulin, a unique feature of the Control-IQ algorithm that can be used simultaneously with the Exercise Activity feature.
Conclusion:
We believe that this expert guidance, with its practical flowcharts, can help simplify decision-making for Control-IQ users.
