Autologous Biofiller in Aesthetic and Regenerative Medicine: A Systematic Review

Introduction:
Autologous biofillers are emerging blood-derived regenerative treatments for soft tissue augmentation and regeneration. Despite increasing use, there is limited consensus regarding their definition, preparation, mechanisms, indications, efficacy, longevity, and safety.
Aims:
To systematically review the literature, providing an evidence-based overview of biofillers and informing clinical practice.
Methods:
A systematic review was performed, and studies evaluating autologous blood-derived biofillers for facial soft tissue augmentation were included. Data on definition, preparation, indications, outcomes, longevity, and safety were synthesised narratively.
Results:
Twenty studies were included. Biofillers were primarily used for facial rejuvenation, infraorbital restoration, wrinkle reduction, and skin improvement. Clinical outcomes and patient satisfaction were favourable with follow-up up to 10 months, while adverse events were predominantly mild and transient. Heterogeneity in terminology, preparation, and outcome reporting precluded meta-analysis.
Conclusions:
Autologous biofiller represents a promising addition to the field of regenerative aesthetic medicine, offering encouraging patient outcomes and a good safety profile. This article provides valuable information supporting clinician and patient decision-making, whilst identifying gaps in the literature.
