PurposeTo present a versatile surgical technique using flanged polypropylene sutures for intrascleral fixation of both intraocular lenses (IOLs) and iris prostheses (IP) in cases of traumatic aphakia and aniridia, conditions frequently observed following penetrating ocular trauma.MethodsThree cases of post-traumatic aphakia and aniridia were managed using flanged polypropylene suture intrascleral fixation of both the IOL and IP. The technique was adapted to the individual anatomical and clinical complexities of each case.ResultsThis technique provided stable and effective positioning of both the IOL and the IP in all three cases. The adaptability of the method facilitated successful visual and cosmetic rehabilitation, despite the presence of additional ocular structural challenges.ConclusionThe flanged intrascleral fixation technique offers a flexible and effective solution for managing complex cases of aphakia and aniridia following penetrating ocular trauma. Further studies with larger cohorts and longer follow-up are warranted to validate the long-term safety, stability, and visual outcomes of these iris prosthesis implantation techniques.
