Rethinking How Research Is Measured in the Otolaryngology Match

Research productivity has become an increasingly important factor in Otolaryngology-Head and Neck Surgery (OHNS) residency selection, particularly following the transition of USMLE Step 1 to pass/fail scoring. In response, the Electronic Residency Application Service (ERAS) will introduce a redesigned "scholarly works" section beginning in the 2027 application cycle that emphasizes peer-reviewed scholarship, consolidates related research outputs, and highlights meaningful contributions. These changes may encourage greater focus on research quality rather than publication volume and improve opportunities for applicants from less research-intensive institutions. However, persistent competitive pressures may limit their impact, as applicants are likely to continue prioritizing activities most valued during the selection process. Additional measures may be needed to reduce emphasis on research quantity and better align incentives with substantive scholarly achievement. Continued evaluation of application trends will be necessary to determine whether these reforms achieve their intended goals.
