[Reducing inappropriate vitamin D testing: an overview]

Abstract:
25-(OH)-vitamin D testing is an example of low-value care whose prevalence has increased dramatically in recent years. In Switzerland, testing rates doubled in less than a decade, resulting in annual costs of over 100 million Swiss francs, without any demonstrated clinical benefit. This article provides an overview of various national and international approaches aimed at reducing low-value care and overtesting. The analysis compares the effectiveness of informative approaches (smarter medicine - Choosing Wisely), decision support approaches (clinical alerts), restrictive approaches (reimbursement limitations), and multimodal approaches. Recent data reveal that no single approach is sufficient: reimbursement restrictions are effective, but erode rapidly; educational recommendations have a modest, yet lasting, impact; multimodal approaches require continuous monitoring.

Role of Skin in Vitamin D Synthesis
The solar UV B rays (290-315 nm) are absorbed by the skin, and 7-dehydrocholesterol (provitamin D3) photolyzes it to previtamin D3, which undergoes a rapid transformation to vitamin D3(cholecalciferol).
