Purpose:
To compare the efficacy, safety, and donor-site morbidity of robot-assisted lingual mucosa graft ureteroplasty (RA-LMGU) versus robot-assisted buccal mucosa graft ureteroplasty (RA-BMGU) for complex ureteral strictures.
Materials And Methods:
We reviewed 149 patients undergoing RA-LMGU (n=71) or RA-BMGU (n=78) across 10 tertiary centers between April 2015 and January 2024. Demographics, stricture characteristics, perioperative outcomes, surgical success, complications, and donor-site morbidity were compared.
Results:
Median stricture length was greater with RA-LMGU (4.0 vs 3.0 cm; p < 0.001). Median operative time (183 vs 185.5 min; adjusted p = 0.219) and median estimated blood loss (40 vs 45 mL; adjusted p = 0.969) did not differ significantly between the RA-LMGU and RA-BMGU groups. At a median follow-up of 25 months, surgical success was 97.2% for RA-LMGU and 96.2% for RA-BMGU (p=1.000). Overall complication rates did not differ significantly (8.5% vs 16.7%; p = 0.133), and no major complications (Clavien-Dindo ≥ III) occurred. Donor-site morbidity revealed site-specific profiles: early eating and drinking difficulties were more frequent after RA-LMGU (15.5% vs 3.8%; p = 0.015), whereas oral tightness (11.5%) and scar-site foreign body sensation (9.0%) were observed only after RA-BMGU. Most symptoms resolved within one month. Among patients with 3-6 cm strictures, the findings were consistent with the overall cohort.
Conclusions:
Both RA-BMGU and RA-LMGU appear safe and effective for complex ureteral strictures, with excellent mid-term outcomes. Although donor-site morbidity profiles differed, most were mild and transient. Prospective multicenter randomized comparative studies are warranted.
