An evidence-informed operational blueprint for delivering focused clinical daily safety huddles in clinical practice
1Senior Lecturer in Nursing, School of Health and Social Care, Sheffield Hallam University.
Abstract:
Despite sustained attention to patient safety, avoidable harm remains prevalent in acute and rehabilitation settings, often arising from communication failures, fragmented teamwork and limited situational awareness. Traditional safety mechanisms are largely reactive and do not consistently support real-time risk management. Focused clinical daily safety huddles (FCDSHs) have emerged as a proactive, nurse-led intervention to close this gap. FCDSHs are structured, multidisciplinary and action oriented, with a focus on identifying and mitigating avoidable patient safety risks. Effective implementation requires preparation, consistent facilitation, multidisciplinary engagement and psychological safety, supported by proportionate documentation and clear escalation processes. Nursing leadership is central to embedding and sustaining FCDSHs within routine clinical practice. Making FCDSHs part of routine care strengthens situational awareness and teamwork while proactively mitigating avoidable patient safety risks.
