Respect My Authority! Nurse Practitioner Leadership and the Problem of Legitimate Authority
1PeaceHealth Medical Group, Oregon, WA, USA.
Abstract:
What makes healthcare authority morally legitimate, and what reasons does informal leadership provide for its institutional recognition? This philosophical analysis examines these questions in the context of nurse practitioner leadership. It distinguishes relational leadership, epistemic standing, formal authorization, and practical authority, and compares its normative purpose with shared, distributed, and collective leadership scholarship. Four South Park characters serve as satirical counterexamples rather than empirical evidence: Cartman represents authorization without justification; Kyle, epistemic standing without decision rights; Butters, participation without meaningful agency; and Kenny, responsibility without adequate resources. Drawing on Weber, Raz, Arendt, Fricker, and Tronto, the analysis develops a provisional account of defensible alignment across five dimensions: function-specific competence, epistemic standing, responsibility and answerability, formal decision rights and resources, and orientation toward patients and collective care. These dimensions organize a burden of justification rather than constitute individually necessary and jointly sufficient conditions or a compensatory score. The contribution lies in their integration for evaluating both the exercise and institutional allocation of authority, not in discovering leadership beyond office. The account rejects professional entitlement for physicians and nurse practitioners alike. It generates a normative question about withholding authority where relevant competence and responsibility are demonstrated, while leaving the existence, prevalence, and consequences of such mismatches to empirical investigation.
