Objectives:
To describe four lesion-based and incision-based quantitative metrics extracted from operative documentation of phonomicrosurgery for a predictive model of vocal fold vibratory outcomes.
Methods:
Four metrics were computed for 30 patients undergoing phonomicrosurgery for benign vocal fold lesions: Incision Size Ratio Index (ISRI), Lesion Area Index (LAI), Incision Position Index (IPI), and operative note score. Associations with postoperative stroboscopic ratings were evaluated using multiple linear regression.
Results:
ISRI was negatively associated with mucosal wave recovery (β = -1.87, P = 0.027), while LAI showed a positive association (β = 13.64, P = 0.026). These effects emerged only in multivariable analysis due to a suppression pattern. IPI and operative note scores showed no predictive value in the current sample.
Conclusion:
This proof-of-concept demonstrates the feasibility of transforming operative documentation into quantitative predictors of surgical outcomes, providing a methodological foundation for future investigation.
