Background:
Abdominal aortic aneurysm (AAA) is a progressive and life-threatening disease, with rupture-related mortality reaching up to 80% in Mexico. Limited national epidemiological data and the need to standardize diagnostic, therapeutic, and follow-up strategies prompted the development of the Second National Consensus on AAA in Mexico, coordinated by the Mexican Society of Angiology, Vascular and Endovascular Surgery (SMACVE), based on the Society for Vascular Surgery guidelines and adapted to local practice.
Methods:
The consensus was developed using the RAND/UCLA Appropriateness Method and a modified Delphi process. A panel of national and international experts evaluated 116 statements derived from Society for Vascular Surgery guidelines. Statements were discussed, reformulated when necessary, and subjected to structured voting. Consensus was defined as ≥ 80% agreement among participants.
Results:
Consensus was achieved for the majority of the evaluated statements, including recommendations on screening, diagnosis, preoperative assessment, medical management, indications for endovascular and open repair, rupture management, perioperative care, follow-up, and organizational aspects. Recommendations were adapted to the clinical practice environment and healthcare resources available within the Mexican healthcare system.
Conclusions:
This consensus provides standardized definitions, diagnostic criteria, and management recommendations for AAA care in Mexico, aiming to promote evidence-based, uniform clinical practice adapted to local healthcare conditions.
