Abstract:
Physiotherapy faces a critical challenge that threatens its scientific credibility and clinical effectiveness: an inadequate grounding in the philosophy of science and logical inference. Although evidence-based practice frameworks are widely adopted, physiotherapists often lack the philosophical foundation to distinguish genuine scientific validation from circular reasoning and theoretical seduction. This philosophical gap fuels recurring "fad cycles" where interventions gain popularity via compelling narratives and testimonials rather than rigorous empirical testing. The issue is not the absence, but the resistance to philosophical inquiry, which mirrors broader trends in healthcare education that marginalize philosophy. Dismissing philosophy as irrelevant overlooks that science itself is philosophical, and every clinical decision requires epistemological judgment about evidence, causation, and certainty. The consequences are far-reaching, compromising patient care through misallocated resources and the delayed adoption of effective treatments. Addressing this crisis requires integrating philosophy of science into physiotherapy education, developing standards for evidence evaluation that stress falsifiability and resistance to disconfirmation, and fostering intellectual humility within the profession. By embracing philosophical sophistication, physiotherapy can move beyond theoretical fads and advance through genuine scientific progress, securing its place as a credible and effective healthcare discipline.
