Abstract:
Compounding medications for intrathecal administration presents distinctive challenges because the preparation is introduced directly into the cerebrospinal fluid and comes into immediate contact with neural and meningeal tissues. Errors involving sterility, bacterial endotoxins, concentration, physicochemical compatibility, excipient selection, or route of administration may result in severe and potentially irreversible neurologic injury or death. Some of the most serious and catastrophic compounding errors reported over the past 25 years have involved preparations intended for intrathecal administration. Pharmacists asked to compound an intrathecal formulation must therefore evaluate not only the scientific feasibility of the preparation but also whether sufficient evidence, highly trained personnel, environmental controls, equipment, and quality systems are available to support its safe preparation. This article discusses fundamental principles associated with intrathecal compounding and considers the pharmacist's ethical responsibility to recognize professional and institutional limitations. When adequate evidence, expertise, or resources are unavailable, referral to a specialized compounding facility may be warranted.
