Localized herpes simplex infections in atopic dermatitis.

J J Leyden, D A Baker

    Archives of Dermatology

    Summary

Localized herpes simplex virus infections in children with atopic dermatitis present with fever and eczema flares. Recurrences suggest reinfection from close contacts, not disseminated disease.

    Area of Science:

    • Pediatric Dermatology
    • Infectious Diseases
    • Virology

    Background:

    • Atopic dermatitis (eczema) is a chronic inflammatory skin condition.
    • Herpes simplex virus (HSV) can cause various skin manifestations.
    • The interaction between HSV and atopic dermatitis requires further clarification.

    Purpose of the Study:

    • To describe the clinical presentation and recurrence patterns of localized herpes simplex virus infections in children with atopic dermatitis.
    • To investigate the potential source of recurrent infections.
    • To differentiate these localized infections from disseminated forms like Kaposi's varicelliform eruption.

    Main Methods:

    • Case documentation of ten children with atopic dermatitis and localized HSV infections.
    • Clinical observation of symptoms including fever, eczema exacerbation, vesicles, and ulcerations.
    • Epidemiological inquiry into recurrent attacks and potential sources of reinfection.
    • Viral cultures from 30 additional patients with atopic dermatitis flares but without vesicular lesions.

    Main Results:

    • Ten children presented with localized HSV infections on atopic dermatitis lesions, characterized by fever, eczema exacerbation, vesicles, and ulcerations.
    • Seven of these children experienced recurrent attacks, often coinciding with herpes labialis in family or friends, indicating reinfection.
    • No disseminated Kaposi's varicelliform eruption was observed in any patient.
    • Viral cultures were negative for HSV in 30 children with atopic dermatitis flares lacking vesicular lesions.

    Conclusions:

    • Localized herpes simplex virus infections can occur in children with atopic dermatitis, presenting distinct clinical features.
    • Reinfection from external sources is a likely cause of recurrent infections in this population.
    • These localized infections differ from the more severe, disseminated forms of HSV in atopic dermatitis.
