Thyroid carcinomas arising in thyroglossal ducts.

C P Page, W T Kemmerer, R C Haff

    Annals of Surgery
    |November 1, 1974

    Summary

Thyroid cancer can arise in thyroglossal duct cysts, often presenting as benign cysts. Papillary thyroid carcinoma is the primary cell type, requiring specific surgical and medical treatments based on disease extent.

    Area of Science:

    • Endocrinology
    • Surgical Oncology
    • Pathology

    Background:

    • Thyroglossal duct cysts (TDCs) are congenital anomalies that can rarely harbor malignancy.
    • Malignant transformation within a TDC is often not recognized until post-operative pathological examination.

    Purpose of the Study:

    • To analyze cases of thyroid carcinoma arising in TDCs.
    • To identify clinical indicators of malignancy in TDC patients.
    • To recommend treatment strategies for these rare tumors.

    Main Methods:

    • Retrospective analysis of six newly reported cases and 60 previously documented cases of thyroid carcinoma in TDCs.
    • Review of clinical presentations, pathological findings, and treatment outcomes.

    Main Results:

    • Papillary thyroid carcinoma was the predominant cell type in all analyzed cases.
    • Preoperative indicators of malignancy were observed in 80% of patients with metastatic disease.
    • Most TDCs with associated malignancy initially presented as benign lesions.

    Conclusions:

    • Thyroid carcinoma within a TDC often requires Sistrunk procedure and thyroid suppression for localized disease.
    • Metastatic cases necessitate comprehensive treatment including resection, hormonal therapy, and lymphadenectomy.
    • Early recognition of potential malignancy in TDCs is crucial for appropriate management.
