Renal adenoma -- a misnomer.

    Summary

    Renal adenomas, previously defined as small kidney tumors, can metastasize. This study suggests discontinuing the term "renal adenoma" due to the risk of metastasis in small renal lesions.

    Background:

    • Histological differentiation between renal cell carcinoma and renal adenoma is challenging.
    • Tumor size (less than 3 cm for adenoma, greater than 3 cm for carcinoma) has been the primary diagnostic criterion.
    • The prevailing concept is that adenomas evolve into adenocarcinomas.

    Observation:

    • This case report and others demonstrate that small renal lesions, previously classified as adenomas, can metastasize.
    • Lesions under 3 cm, typically labeled as adenomas, rarely metastasize but are not entirely risk-free.

    Findings:

    • Metastasis from small renal lesions (<3 cm) can occur, challenging the traditional size-based classification.
    • The histological distinction between benign adenoma and malignant carcinoma in the kidney is not reliably size-dependent.

    Implications:

    • The term "renal adenoma" should be reconsidered and potentially abandoned in clinical practice.
    • Rethinking diagnostic criteria for renal tumors is necessary to account for the metastatic potential of smaller lesions.
    • Clinical management and surveillance strategies for small renal masses may need revision.
