Objective:
To evaluate postoperative outcomes of patients who underwent same-day discharge to a hybrid Care Hotel recovery model following robotic-assisted radical prostatectomy (RARP).
Methods:
The Care Hotel is an outpatient postoperative recovery unit that provides virtual and in-person care as an alternative to overnight hospital admission. The primary outcome was 30-day hospital readmission and secondary outcomes were Clavien-Dindo complication rates, 48-hour emergency department (ED) visits, and 30-day ED visits. Demographic characteristics, American Society of Anesthesiologists (ASA) class, operation duration, and Care Hotel call frequency and duration were analyzed for associations with outcomes.
Results:
Between August 2021 and August 2023, 174 patients underwent RARP with same-day discharge to the Care Hotel. Almost three-quarters of patients (74.2%) travelled >50 miles for surgery. The 30-day hospital admission rate was 2.3%. The overall Clavien-Dindo complication rate was 9.2%, with all complications classified as Grade 1 (81%) or Grade 2 (19%). 48-hour and 30-day ED visit rates were 2.3% and 8.0%, respectively. Nearly half of patients (48.9%) initiated at least one Care Hotel call, with a median duration of 1.6 minutes. No statistically significant associations were identified between postoperative outcomes and demographic variables, ASA class, or operation duration.
Conclusions:
Same-day discharge to the Care Hotel following RARP was associated with low readmission, ED utilization, and complication rates. These findings support the Care Hotel as a safe alternative to inpatient admission that may optimize postoperative care and resource utilization.
