    Summary

    Enucleation for small choroidal melanomas showed lower tumor-related death rates than previously reported. Observing small melanomas (under 98 cu mm) did not increase metastatic melanoma risk.

    Background:

    • Choroidal melanomas are the most common primary intraocular malignancy.
    • Enucleation has been a standard treatment for choroidal melanomas, but its impact on long-term mortality for small tumors is debated.

    Purpose of the Study:

    • To evaluate the long-term tumor-related mortality in patients who underwent enucleation for small choroidal melanomas.
    • To compare mortality rates with historical data and assess the risk associated with tumor size and observation.

    Main Methods:

    • Retrospective analysis of 65 patients with enucleated eyes due to small choroidal melanomas (≤300 cu mm).
    • Follow-up data collected over 3 to 34 years.
    • Tumor volumes were analyzed in relation to metastatic death outcomes.

    Main Results:

    • 5-, 10-, and 15-year tumor-related death rates were lower than previously reported for small melanomas post-enucleation.
    • Mortality rates were comparable to estimated rates in patients not undergoing enucleation.
    • Tumors causing metastatic death had an average volume more than double those that did not.
    • No metastatic deaths occurred in patients with tumors <98 cu mm.

    Conclusions:

    • Enucleation for small choroidal melanomas is associated with favorable long-term survival.
    • Observation of small choroidal melanomas (<98 cu mm) appears safe and does not elevate the risk of metastatic death.
