    Background:

    • Patients with chronic renal failure (CRF) exhibit diverse renal diseases.
    • Azotaemic renal osteodystrophy encompasses osteitis fibrosa, osteomalacia, and osteosclerosis.

    Purpose of the Study:

    • To describe the histopathology of bone in patients with CRF.
    • To quantitatively assess bone changes and their relationship with secondary hyperparathyroidism.

    Main Methods:

    • Histopathological examination of bone in 60 CRF patients.
    • Quantitative histology using point-counting technique.
    • Comparison with 68 healthy individuals.

    Main Results:

    • Significant increases in total bone, mineralized bone, and osteoid in CRF patients.
    • Osteitis fibrosa (93%), osteomalacia (40%), and osteosclerosis (30%) were prevalent.
    • Woven bone formation correlated with osteitis fibrosa severity and parathyroid gland weight.

    Conclusions:

    • Hyperparathyroidism and osteitis fibrosa are early complications of CRF.
    • Osteomalacia develops later in the disease progression.
    • Woven bone formation and excess osteoid contribute to osteosclerosis in CRF.
