Background:

Parathyroid cysts are rare, often presenting as neck masses.
Hypercalcemia and elevated parathyroid hormone (PTH) levels are key indicators of parathyroid pathology.

Observation:

A large cervicomediastinal parathyroid cyst was identified.
The cyst was associated with significant hypercalcemia and elevated serum PTH levels.

Findings:

Hemorrhage into a parathyroid adenoma is the most probable cause of this large cyst.
High concentrations of PTH were detected within the cyst fluid.

Implications:

Cystic masses at the root of the neck warrant serum calcium assessment.
Measuring PTH in cyst aspirates can aid in diagnosing parathyroid cysts.
