    Summary

    A well-trained man experienced convulsions during prolonged exercise in heat, linked to low serum magnesium. Supplementation and heat acclimatization resolved the issue, preventing recurrence during subsequent exercise.

    • Treatment with phenobarbital and magnesium chloride supplementation normalized serum magnesium levels.
    • The subject successfully completed heat acclimatization and repeated exercise tests without adverse effects.

    • Low serum magnesium may be a critical factor in exercise-induced seizures during heat exposure.
    • Magnesium supplementation and heat acclimatization are potential strategies for prevention.
