    Subclinical cardiovascular disease in patients with chronic obstructive pulmonary disease: a systematic review.

    Background:

    • Phenformin, a biguanide antihyperglycemic agent, can induce lactic acidosis.
    • Lactic acidosis is a critical condition characterized by lactate accumulation and acidemia.
    • Understanding the hemodynamic consequences of phenformin-induced lactic acidosis is crucial for patient management.

    Observation:

    • Two patients with phenformin-related lactic acidosis exhibited severe acidemia (pH 7.05-6.80) and markedly elevated blood lactate (11.5-26.1 mM/L).
    • Hemodynamic studies revealed significantly reduced cardiac indices (0.94-1.15 L/min/m2), stroke volume, and stroke work, with normal to increased arterial resistance.
    • Pulmonary artery pressures were mildly elevated, but necropsy ruled out pulmonary vascular obstruction. Cardiac failure and hypovolemia were excluded as primary causes.

    Findings:

    • The low cardiac output state was not attributable to primary cardiac failure or hypovolemia.
    • Oxygen extraction was not significantly impaired, suggesting oxygen utilization was preserved.
    • The primary circulatory defect appears to be impaired oxygen delivery due to an intravascular blood volume distribution issue.

    Implications:

    • Phenformin-related lactic acidosis can manifest as a shock state characterized by impaired oxygen delivery.
    • The findings suggest a critical role for intravascular fluid distribution in the pathophysiology of this condition.
    • This highlights the need for careful hemodynamic monitoring and management strategies focused on circulatory support in phenformin-induced lactic acidosis.
