    Summary

    Chronic hypercapnia can cause papilledema and optic atrophy in patients. Raised intracranial pressure due to cerebral edema is the likely cause, especially with prolonged symptoms.

    Background:

    • Chronic hypercapnia, a condition of prolonged elevated carbon dioxide levels in the blood, presents complex neurological and ophthalmological challenges.
    • Understanding the ocular manifestations of chronic hypercapnia is crucial for patient management.

    Observation:

    • Two patients with 10 and 13 years of hypercapnia presented with papilledema.
    • One patient progressed to optic atrophy, indicating potential long-term visual impairment.

    Findings:

    • Papilledema in hypercapnia may result from local retinal vascular changes, generalized cerebral vasculature alterations, or elevated intracranial pressure.
    • Computerized axial tomography in one case suggested cerebral edema, supporting raised intracranial pressure as a significant factor.
    • Skull radiographic changes associated with hypercapnia have not been previously documented.

    Implications:

    • The duration of papilledema appears critical for the development of optic atrophy.
    • Cerebral edema and subsequent increased intracranial pressure are strongly implicated in the pathogenesis of papilledema in hypercapnic patients.
    • Further research is needed to elucidate the precise mechanisms linking hypercapnia to these ocular findings.
