    Background:

    • Parkinson disease (PD) is a neurodegenerative disorder primarily affecting motor function.
    • Dementia is a common non-motor symptom in PD, impacting quality of life.
    • The neuropathological basis for dementia in PD is not fully understood.

    Purpose of the Study:

    • To investigate the neuropathological overlap between Parkinson disease and Alzheimer disease.
    • To determine the prevalence of Alzheimer disease neuropathology in Parkinson disease patients with dementia.

    Main Methods:

    • Retrospective analysis of 34 autopsy cases of Parkinson disease (PD) and age/sex-matched controls.
    • Neuropathological examination of brains for hallmarks of Alzheimer disease (AD), including plaques and tangles.
    • Review of clinical data to assess the presence and degree of dementia.

    Main Results:

    • 56% of Parkinson disease cases exhibited some degree of dementia.
    • Parkinson disease brains showed significantly reduced weight compared to controls (p < 0.02).
    • Widespread Alzheimer disease neuropathology (plaques, tangles, granulovacuolar degeneration, cortical cell loss) was found in most PD brains, and to a lesser extent in controls.

    Conclusions:

    • The high incidence of dementia in Parkinson disease patients is likely attributable to the co-occurrence of Alzheimer disease neuropathology.
    • Simultaneous neuropathological changes of PD and AD may explain the cognitive decline observed in PD patients.
    • Further research is warranted to explore the mechanisms underlying the interaction between these two neurodegenerative diseases.
