    Background:

    • Cardiac arrhythmias, including sinus node and atrioventricular (AV) node disease, affect heart rate and conduction.
    • Beta-adrenergic stimulation is a potential therapeutic strategy for certain cardiac rhythm disorders.

    Purpose of the Study:

    • To evaluate the electrophysiological effects of prenalterol, a beta-stimulator, on cardiac conduction in patients with diverse arrhythmias.
    • To assess the potential clinical utility of prenalterol in managing bradycardia and pacemaker failure.

    Main Methods:

    • Electrophysiological studies were conducted in 19 patients diagnosed with sinus node disease, AV node disease, double node disease, or conduction disturbances.
    • Measurements included heart rate, sinus node recovery time, AH interval, and functional refractory period of AV conduction.

    Main Results:

    • Prenalterol uniformly improved sinus node function, as indicated by heart rate and sinus node recovery time.
    • Significant and uniform improvement in AV conduction was observed, with shortened AH intervals and functional refractory periods.
    • Beta-stimulation with prenalterol increased spontaneous depolarization in His-Purkinje fibers, leading to shorter escape intervals and higher escape rhythm frequency in patients with complete AV block.

    Conclusions:

    • Prenalterol effectively enhances sinus node function and AV conduction.
    • The drug may be clinically beneficial for patients with cardiomyopathies experiencing bradycardia or those with severe bradyarrhythmias.
    • Prenalterol shows potential utility in managing complete pacemaker failure and in emergency situations.
