    Background:

    • Sjögren's syndrome (SjS) is a chronic autoimmune disease primarily affecting exocrine glands.
    • The role of circulating immune complexes (IC) in SjS pathogenesis and systemic involvement remains an area of investigation.

    Purpose of the Study:

    • To investigate the presence and significance of circulating immune complexes (IC) in patients with Sjögren's syndrome (SjS).
    • To correlate IC levels with disease activity, systemic manifestations, and specific clinical features in SjS.

    Main Methods:

    • Utilized the 125I-Clq binding assay to detect and quantify circulating immune complexes (IC) in Sjögren's syndrome (SjS) patients.
    • Correlated IC detection with clinical data, including presence of systemic lupus erythematosus (SLE), other associated diseases, erythrocyte sedimentation rate (ESR), serum gammaglobulin levels, and sialographical findings.

    Main Results:

    • Circulating immune complexes (IC) were detected in 52% of Sjögren's syndrome (SjS) patients.
    • A high incidence of IC (80%) was observed in SjS patients with co-existing systemic lupus erythematosus (SLE).
    • IC were present even in 40% of SjS patients without other defined diseases, and their levels correlated positively with ESR, serum gammaglobulin, and severity of parotid gland sialographical changes.

    Conclusions:

    • Circulating immune complexes (IC) are frequently present in Sjögren's syndrome (SjS) patients, irrespective of associated diseases.
    • The presence and levels of IC are associated with systemic manifestations and disease severity in SjS.
    • These findings support the role of circulating IC in the advancement and systemic involvement of Sjögren's syndrome.
