    Background:

    • Pseudohypoparathyroidism is a condition characterized by resistance to parathyroid hormone (PTH).
    • Patients may show partial responses to exogenous PTH extracts.

    Observation:

    • A patient with pseudohypoparathyroidism developed resistance to PTH extract after repeated administration.
    • Serum analysis revealed the presence of gamma G globulin antibodies that bound to labeled bovine PTH.

    Findings:

    • These antibodies inhibited the in vitro activation of adenylate cyclase by PTH.
    • The antibodies were primarily directed against the C-terminal portion of the PTH molecule.

    Implications:

    • This research attributes clinical resistance to parathyroid hormone to specific autoantibodies.
    • Identifies a potential mechanism for treatment unresponsiveness in pseudohypoparathyroidism.
