    Background:

    • Acral syringoma is a benign adnexal tumor originating from sweat glands.
    • Its histogenesis remains debated, with varying proposed origins.

    Observation:

    • A case of acral syringoma presented with unusual eruption sites alongside typical lower eyelid involvement.
    • Light microscopy revealed solid tumor cell nests and cystic formations in the upper dermis.
    • Electron microscopy identified ductal structures within solid nests, characterized by microvilli and luminal cell shedding.

    Findings:

    • Tumor cells exhibited features suggestive of eccrine differentiation, including duct formation and specific cytoplasmic inclusions.
    • Multivesicular bodies (mvb) and keratohyalin granules were prominent in periluminal tumor cells.
    • Observed ductal structures bore resemblance to embryonic intra-epidermal ducts.

    Implications:

    • The findings support an origin of acral syringoma from the eccrine intra-epidermal duct.
    • This study provides novel ultrastructural evidence contributing to the understanding of syringoma histogenesis.
    • Further research may elucidate the specific developmental pathway involved in acral syringoma formation.
