    Background:

    • Human Leukocyte Antigen (HLA) typing is crucial for transplantation and disease association studies.
    • Monocytes and lymphocytes are key immune cells expressing HLA determinants.

    Purpose of the Study:

    • To evaluate the efficacy of using monocyte-enriched cell suspensions for HLA-A, -B, and -DR typing.
    • To compare HLA typing results from monocytes with those from autologous lymphocytes and B-lymphocytes.

    Main Methods:

    • Monocyte-enriched cell suspensions were prepared from 57 blood donors using a plate adherence method.
    • Microcytotoxicity assays were performed on monocytes, unfractionated lymphocytes, and B-lymphocytes for HLA typing.
    • Correlation coefficients were calculated to assess the reliability of monocyte-based HLA typing.

    Main Results:

    • High correlation coefficients (r > 0.79 for HLA-A, r > 0.56 for HLA-B, r > 0.81 for HLA-DR) were observed.
    • Detection of certain HLA-A and -B specificities was less reliable on monocytes compared to lymphocytes.
    • Monocytes and B-cells demonstrated nearly identical strength and reliability for HLA-DR typing.

    Conclusions:

    • Monocytes are a viable option for HLA-DR typing, offering comparable results to B-lymphocytes.
    • Monocyte-based typing for HLA-A and -B may require further optimization due to variable detection sensitivity.
