    Postoperative elevations in serum amylase are common after cardiac surgery with cardiopulmonary bypass. However, these increases are typically not due to pancreatic issues and do not indicate pancreatitis.

    Background:

    • Postpump pancreatitis is a potential complication following cardiac surgery involving cardiopulmonary bypass.
    • Elevated serum amylase levels are sometimes observed in these patients.

    Purpose of the Study:

    • To investigate the incidence and source of hyperamylasemia in patients after cardiopulmonary bypass.
    • To determine if postoperative hyperamylasemia is indicative of clinical pancreatitis.

    Main Methods:

    • Prospective analysis of 20 patients undergoing cardiac surgery with cardiopulmonary bypass.
    • Measurement of total serum amylase, pancreatic isoamylase, and nonpancreatic isoamylase levels postoperatively.

    Main Results:

    • Six of 19 evaluable patients exhibited postoperative hyperamylasemia.
    • The majority of hyperamylasemia was attributed to nonpancreatic isoamylase, not pancreatic isoamylase.
    • No patient presented with clinical signs or symptoms of pancreatitis.

    Conclusions:

    • Elevated serum amylase is a frequent finding after cardiopulmonary bypass.
    • Postoperative hyperamylasemia following cardiopulmonary bypass is generally not indicative of pancreatitis.
