This study reports the first case of pacemaker tachycardia triggered by myopotentials in a patient with a DDD,M pacemaker programmed to DVI mode. Reprogramming was required to terminate the tachycardia episodes.

    Area of Science:

    • Cardiology
    • Biomedical Engineering

    Background:

    • Ventricular tachycardia (VT) is a serious arrhythmia often requiring pacemaker implantation.
    • Pacemakers, such as the DDD,M device used here, can sometimes lead to pacemaker-mediated tachycardia (PMT).

    Observation:

    • A 67-year-old male with drug-refractory VT received a DDD,M pacemaker programmed to DVI mode.
    • The patient experienced two episodes of 150 bpm tachycardia, identified as PMT.
    • PMT was easily provoked by pectoral muscle contraction, indicating myopotential interference.

    Findings:

    • This is the first reported instance of PMT induced by myopotentials in a pacemaker programmed to DVI mode.
    • The tachycardia episodes required intervention, with one episode self-terminating and the second necessitating pulse generator reprogramming.

    Related Experiment Videos

    Implications:

    • Myopotential interference should be considered in patients experiencing PMT, especially with DVI mode programming.
    • Understanding pacemaker-myopotential interactions is crucial for optimizing device programming and patient management.
    • This case highlights the importance of comprehensive troubleshooting for pacemaker-related arrhythmias.
