Background:

The Diagnostic and Statistical Manual of Mental Disorders, Third Edition (DSM-III) introduced the schizophreniform disorder category.
Previous research has identified demographic, clinical, and laboratory variables to differentiate schizophrenia from affective disorders.
Understanding the boundaries between psychotic and mood disorders is crucial for accurate diagnosis and treatment.

Purpose of the Study:

To investigate the characteristics of manic patients who also meet DSM-III schizophreniform criteria.
To compare these patients with manics who do not meet schizophreniform criteria.
To assess the validity of the DSM-III schizophreniform category in psychiatric research.

Main Methods:

Applied DSM-III schizophreniform criteria to 111 patients diagnosed with mania using Feighner criteria.
Compared 13 patients (12%) meeting DSM-III schizophreniform criteria to the remaining manic patients.
Analyzed demographic, clinical, laboratory variables, and family history of psychiatric disorders.

Main Results:

Manics meeting DSM-III schizophreniform criteria exhibited more depressive symptoms than other manics.
No significant differences were found between the groups in demographic, clinical, or laboratory variables previously used to distinguish schizophrenia from affective disorders.
The group meeting schizophreniform criteria had a lower morbid risk for affective disorder but twice the morbid risk for alcoholism in relatives compared to other manics.

Conclusions:

The DSM-III schizophreniform category may include individuals with features overlapping with affective disorders.
The findings raise questions about the distinctiveness and research validity of the DSM-III schizophreniform diagnosis.
Further research is needed to clarify the nosological status of schizophreniform disorder and its relationship to mood and substance use disorders.
