    Background:

    • Severe acne vulgaris affects numerous patients, often necessitating effective therapeutic interventions.
    • Cyproterone acetate is a known anti-androgen used in acne treatment.

    Purpose of the Study:

    • To compare the efficacy of high-dose versus low-dose cyproterone acetate for severe acne.
    • To evaluate the impact of topical treatments when used with cyproterone acetate.

    Main Methods:

    • 144 patients with severe acne were treated with either low-dose Diane (2 mg/day for 21 days) or high-dose Androcur (50-100 mg/day for 10 days) in an inverse sequential schedule.
    • Efficacy was assessed based on seborrhea, comedones, and eruptions.
    • The study also assessed outcomes with and without adjunctive topical treatments (Tretinoin or Benzoyl peroxide).

    Main Results:

    • High-dose Androcur demonstrated a highly significant improvement in seborrhea, comedones, and eruptions compared to low-dose Diane.
    • The efficacy of Androcur was consistent regardless of whether 50 mg or 100 mg was administered.
    • Adjunctive topical treatments did not significantly alter the positive outcomes observed with Androcur.

    Conclusions:

    • High-dose cyproterone acetate (Androcur) is a more effective treatment for severe acne vulgaris than low-dose cyproterone acetate (Diane).
    • Androcur shows significant efficacy in managing seborrhea, comedones, and inflammatory lesions.
    • The drug is well-tolerated, and its effectiveness is not diminished by concurrent topical therapies.
