Background:

This study evaluates the long-term efficacy of the Bankart repair for anterior shoulder instability.
The research spans a thirty-year period, analyzing surgical outcomes and patient-reported results.

Observation:

Surgical findings included anterior capsule separation (85%), Hill-Sachs lesions (77%), and anterior glenoid rim damage (73%).
Recurrence rate after repair was 3.5% in 145 followed shoulders.
98% of patients reported excellent or good outcomes, with 69% achieving full range of motion.

Findings:

A meticulous Bankart repair technique demonstrated a low recurrence rate of 3.5%.
Glenoid rim fractures did not elevate recurrence risk, while moderate-to-severe Hill-Sachs lesions slightly increased it.
Excellent or good results were reported by 98% of patients, with most returning to prior athletic activities.

Implications:

The Bankart repair allows for early return to motion and function without the need for postoperative immobilization.
This surgical approach facilitates a high rate of return to competitive athletics with no limitations in shoulder motion.
The findings support the Bankart repair as a reliable and effective treatment for anterior shoulder instability.
