    Summary

    A patient experienced chronic respiratory failure due to abnormal breathing patterns during sleep. Diaphragmatic pacing corrected this, resolving the failure and highlighting the importance of sleep breathing patterns.

    Background:

    • Investigated a 67-year-old man with a 4-year history of chronic ventilatory failure.
    • Ruled out restrictive/obstructive defects and performed unremarkable neurologic investigations.
    • Confirmed normal minute ventilation response to inhaled CO2 during wakefulness.

    Observation:

    • During sleep, the patient exhibited a rapid, shallow breathing pattern.
    • Minute ventilation during sleep (6.7 L/min) was comparable to wakefulness (7.2 L/min).
    • No evidence of upper airway obstruction was detected during sleep.

    Findings:

    • Sleep-associated abnormal breathing pattern led to increased dead space ventilation.
    • Inadequate alveolar ventilation resulted in hypoxemia and hypercapnia during sleep.
