    Summary

    Diabetes due to chronic pancreatitis is increasingly common. Treatments for chronic pancreatitis do not prevent or treat the associated diabetes, which can lead to severe complications.

    Context:

    • Chronic pancreatitis is a growing concern, linked to increased alcohol consumption and enhanced diagnostic capabilities.
    • Diabetes mellitus secondary to chronic pancreatitis is a significant and increasingly recognized clinical issue.

    Purpose:

    • To investigate the prevalence and progression of diabetes in patients with chronic pancreatitis.
    • To assess the efficacy of medical and surgical treatments for chronic pancreatitis on the development and management of secondary diabetes.

    Summary:

    • A study of 179 chronic pancreatitis patients (1970-1981) revealed a high incidence of glucose intolerance and diabetes.
    • At admission, 23% had hyperglycemia, and during follow-up, 27 developed clinical diabetes, with 3 deaths from hyperosmolar coma.
    • Long-term observation showed 55% with abnormal fasting glycemia and significant rates of vascular, cardiac, and renal complications.

    Impact:

    • Treatments for chronic pancreatitis symptoms do not appear to halt the progression of secondary diabetes.
    • Patients with chronic pancreatitis are prone to insulin dependence, metabolic decompensation, and severe long-term diabetic complications.
